1. Acceptance of Terms

By accessing or using FormChek ("the Platform"), you agree to be bound by these Terms of Service. If you do not agree to these terms, you may not use the Platform. FormChek reserves the right to update these terms at any time. Continued use of the Platform after changes constitutes acceptance of the revised terms.

2. Description of Service

FormChek is an online marketplace that connects athletes with professional coaches. Athletes submit training videos and receive personalized feedback from coaches through subscription plans or one-time review purchases. FormChek facilitates the transaction but does not directly provide coaching services.

3. User Accounts

You must provide accurate, complete information when creating an account. You are responsible for maintaining the confidentiality of your login credentials and for all activities under your account. You must be at least 18 years old to use FormChek. If you are under 18, you may only use the Platform with the involvement and consent of a parent or legal guardian.

4. Subscriptions & Payments

Subscription plans auto-renew on a monthly basis unless canceled before the renewal date. You may cancel your subscription at any time through your dashboard. Cancellation takes effect at the end of the current billing period — you will retain access until then. One-time single review purchases are charged immediately upon checkout.

5. Refund Policy

All purchases on FormChek are final and non-refundable, with the following exception:

Undelivered reviews: If a coach fails to provide a review for a submitted video within 14 days of submission, you may request a full refund for that specific review. Refund requests for undelivered reviews must be submitted to our support team at support@formchek.com.

Refunds are not provided for: dissatisfaction with feedback quality, unused video submission credits, partial billing periods after cancellation, or change of mind after purchase.

6. Coach Responsibilities

Coaches on FormChek are independent contractors, not employees of FormChek. Coaches are responsible for providing timely, honest, and professional feedback on submitted videos. Coaches must not share athlete-submitted content without explicit consent. FormChek reserves the right to remove coaches who consistently fail to deliver reviews or violate platform guidelines.

7. Athlete Responsibilities

Athletes must only submit videos that they have the right to share. Submitted content must relate to athletic training and must not contain inappropriate, illegal, or offensive material. Athletes must not harass, threaten, or abuse coaches through the platform.

8. Content Ownership

Athletes retain ownership of their submitted videos. Coaches retain ownership of their written and video feedback. By using FormChek, you grant us a limited, non-exclusive license to store, process, and display your content as necessary to operate the Platform.

9. Prohibited Conduct

Impersonating another person or entity
Circumventing platform fees or payment systems
Uploading malicious software or content
Scraping or automated access to the Platform
Sharing account credentials with third parties
Using the Platform for any illegal purpose

10. Limitation of Liability

FormChek is provided "as is" without warranties of any kind. We are not liable for the quality, accuracy, or completeness of any coaching feedback provided through the Platform. FormChek is not responsible for injuries resulting from following coaching advice. In no event shall FormChek's total liability exceed the amount you paid in the 12 months preceding the claim.

11. Termination

We reserve the right to suspend or terminate your account at our discretion, with or without notice, for conduct that violates these Terms or is harmful to other users, coaches, or the Platform.
